In order to ensure your privacy and to protect your membership investment,
we will have a representative call you (or, at your option, email you)
to give you your current user ID and password. Please provide your name
and a number where we can reach you in the form below so that we can
get the information to you quickly.
When we call you, we will ask for your COSE
member ID (available on your member certificate). We may also ask
you to confirm basic information about your company or organization in
order to prevent unauthorized persons from obtaining your password.
